Pharmacy sales accelerate

Poland’s medical-cannabis industry has developed into a regulated pharmaceutical market worth several hundred million zlotys a year. Bankier.pl reports that the country is now Europe’s third-largest medical-cannabis market and the second largest in the European Union, behind Germany. The United Kingdom is the other European market ahead of Poland.

Gross quarterly retail sales through pharmacies increased from PLN 38.0 million in the second quarter of 2023 to almost PLN 130 million in the second quarter of 2026. Growth accelerated in the second half of 2025: sales rose from PLN 73.1 million in the second quarter to PLN 106.9 million in the third, a 46% quarter-on-quarter increase, before reaching PLN 119.8 million in the fourth quarter.

During the first half of 2026, the market generated PLN 250.2 million in gross sales, up 84% year on year. Gross sales over the latest four quarters reached PLN 500 million. Based on average pharmacy prices, pharmacists dispensed an estimated 10–12 tonnes of pharmaceutical cannabis material during that period.

Lower prices broaden patient access

A gram of medical cannabis generally costs PLN 15–45 at a Polish pharmacy, compared with PLN 45–80 several years ago. The decline reflects greater supply and competition. An average prescription is estimated to contain 10 grams, although monthly use varies substantially by condition, from 1 gram for some patients to 30 grams for others.

The number of active patients has exceeded 100,000. Bankier.pl links continued growth to increasing awareness among patients, primary-care doctors and specialists treating chronic pain and insomnia or supporting oncology therapies.

Regulatory changes introduced at the end of 2024 restricted remote consultations and caused a temporary contraction. According to Bankier.pl, the measures removed speculative operators and shifted treatment toward a more rigorous clinical pathway. Demand subsequently recovered, with turnover reaching record levels by the end of 2025.

Distribution and compliance shape competition

The market currently has 12 active marketing-authorisation holders and six distributors offering several dozen registered products. Despite gradual fragmentation, the two largest operators still account for more than 70% of turnover. Compliance with standards enforced by Poland’s Office for Registration of Medicinal Products, including Good Manufacturing Practice and Good Distribution Practice, creates a substantial barrier to entry.

NewConnect-listed Cosma illustrates the commercial opportunity. Its consolidated net revenue reached PLN 10.59 million in the first half of 2026, nearly 74% higher year on year. The group reported a PLN 120,900 net profit, reversing a loss of more than PLN 4.29 million in the corresponding period of 2025. The improvement followed commercial sales of Cannabis flos COSMA THC 22%, CBD ≤1% and an increase in registered pharmaceutical materials from two to four.

Cosma’s second-quarter revenue rose by more than 130% year on year to PLN 7.28 million, from PLN 3.16 million. Profit on sales was PLN 2.06 million, against a PLN 1.19 million loss a year earlier, while quarterly net profit reached PLN 1.91 million and the group’s net margin exceeded 26%. The company attributes greater product availability partly to its partnership with Spanish producer Linneo Health. Its own pharmaceutical wholesaler reduces reliance on intermediaries, while its supplement and cosmetics business supplies more than 1,000 retail locations. For market participants, stable pharmaceutical-grade supply, competitive pharmacy pricing and the lengthy product-registration process will remain central to growth.
